For no apparent reason, I had the following event four times today doing Nihlathak runs:
-Tomes of Identification
-Tomes of Town Portals
-Keys
-and random Charms
Disappeared in my inventory.

UPDATE:
A joint effort suggests that the upper limit of 8192bytes in the d2s savefile is the reason for the disappearing items. It looks like the file upper limit is the same as in d2 original, see:

With the increased size of the stash, it’s possible to possess up to 12 items more than the file is capable of saving. Those items then get lost upon leaving the current game.
Decay from planetdiablo tested out a maximum load of items on a character in single player by loading all-6-affix rings and amulets into stash and inventory. He found out that the d2s savefile limit is exactly the same as in Diablo2 orginal, so 8192bytes. See here:

Together with the increased stash size of Diablo2 Resurrected, this can lead to a situation in which not all items can be saved properly. He tried it out twice with losing 12 items the first time and 11 items the second trial.
