we are already pretty far away from whatever vanilla was with new runes, abilities, dual spec incoming, world buffs (new ones too!) and the various other changes like the pvp ranking change to the addition of the incubus.

If you equip skull, it gets put on a 30 second internal cooldown before you can use it. So someone using it already has it equipped. It is not uncommon.
