There’s a carving of a mural in Khaz Algar that depicts what looks like an old god. Multiple eyes, tentacles, and gigantic. Could it be her? (check @haughtychicken on x for the screenshots)

We don’t know for sure if the 5th was G’huun. They were a creation of the titans.

Pretty sure one of the WoW Chronicles hint at a fifth old god where it’s shown aside the original four. Possibly from the void? We do know there are many other old gods out there.
We can assume it is though, cause chronicle, from the titan PoV, says they sealed 4 evils within the earth.
They sealed G’huun, which implies he was the 4th.
Those look completely different. The mouth is N’zoth’s most prominent feature.
