Consistent Disconnects

During some games I disconnect part of the way through. I would say 1 in 3 games this issue occurs. The game will then say reconnecting and take a nice bit of time to reconnect if it successful at all. However, if I quit and join game in progress it takes just a little bit to reconnect.

I ran Blizzard looking glass at the time this occurred and here are the results

I see the 100% packet loss and was wondering if it might have something to do with that, but after trying all of the trouble shooting recommendations I am unsure of where to go from here.
