I’ve played hearthstone since launch and I have several characters in WOW that are well above lvl 20. Why do I not have Liadrin?
Lady Liadrin is not retroactive to having level 20 plus characters. You just need to raise any sub-20 character to 20.

But I have. More than once since Hearthstone started and the incentive was introduced. That’s why I’m confused.
Are Wow and Hearthstone on the same Blizzard Battle.net account?
Do you have the Fledgling Hero of Warcraft achievement (under Feats of Strength -> Promotions) on your WoW account?
Have you leveled a character to 20, not 19 to 21, but to 20?
Not sure if the level squish in Wow has changed this.
