I’ve probably done 100+ runs of van4 or higher at the moment and these are issues that constantly occur (both to me and the people I am running with). They make it very difficult to enjoy this mode properly and if people aren’t aware of the workarounds for these bugs it can make things pretty frustrating.
Invisible explosions
Mobs with the exploding affix sometimes have invisible explosion graphics (you can’t see the circle but the explosion still happens). This has killed many people in my vanguard groups, they don’t see the circle (and I don’t either, but I know to keep my distance to stay alive). This is mainly an issue on the last barricade as the explosions are biggest there and hurt the most. It is really frustrating to fail a vanguard run near the end because your group died to explosions no one could see.
Incoming monsters warning is not always displayed
This makes it difficult for people to dodge the subsequent red circles if they aren’t super-focused. This can be fixed by logging out of the game and back in again.
Conjuration of light doesn’t always work
Occasionally I will press conjuration of light, the skill will go on cooldown, but no shield will be applied to anyone. The ‘workaround’ here is simply not to rely on shields to save your group and treat it more as a backup.
Enter screen bugs

When you have level 4 difficulty selected and press enter, sometimes the confirmation screen text says you have selected “Elite” difficulty instead, and it will actually take you into elite difficulty if everyone else hits accept. It’s really frustrating when you have checked you have level 4 selected before hitting ‘Enter’ but the confirmation text says ‘Elite’. And then you cancel and make sure that level 4 is selected again before hitting ‘Enter’ and it still says ‘Elite’. I have noticed that trying to select other difficulties (eg. 2 and 3) before selecting level 4 again seems to fix this. But again I’m not sure that most people are aware of this, possibly they just go into elite difficulty thinking it’s level 4, and if they fail they might not realise it’s because it was the wrong difficulty.

Also note that at other times, it will display “Level 4” for a split second after pressing enter and then the text will switch to “Elite”- when this happens, if everyone hits accept it will usually take you into level 4 difficulty. (In other words, if it says ‘Elite’ immediately you need to cancel and try selecting other difficulties before selecting level 4 again, but if it says ‘Level 4’ then the text changes to ‘Elite’ it’s usually fine to go in.)
