Not sure why I can’t move to Inferno 1

I have defeated Iscea, twofold rebirth several times but I’m unable to unlock inferno 1. Any insight as to why I can’t?

To unlock access to Inferno 1, you need to kill the 5th boss of Hell 3, i.e. Peisthal.

Iscea is the 4th boss of Hell 3.

This is not a bug.
