For whatever reasons, multiple mouse and tried the default mouse over and a macro mouse over, that little mouse button when you click the wheel, WoW does not like it, it will not activate things like PW:S or a cleanse, etc. it does not like it, for whatever reason.
I’m not sure. I haven’t had an issue with mine. I use the coveted and now discontinued Logitech G600.
It could be something in the mouse software?

I got my middle mouse bound to jump so i can do all my movement with the mouse alone and I have never had it not work on any of the mice I’ve used over the years.
What mouse are you using? Do you have anything bound to the scroll function of the wheel? Maybe when you push to use the button it’s registering it as a scroll instead?
I dont use mouse over macros so i cant speak to that but my mouse wheel is my interrupt and it works like a charm.
my middle mouse click works just fine, use it for Heroic leap on warrior, Hunter harpoon, Rogue harpoon, Mage Blink etc etc

PW:S has been bound to my mouse wheel button for over a decade and I’ve never had an issue
My mouse wheel click activates my flying mount, and has been since the original BC released. Make sure the wheel itself is set to “click scroll” instead of smoothly scrolling. If it’s set to the latter, it’s easy to slightly slip and the game will think you’re scrolling rather than clicking.

So I need to make it very clear…MOUSE OVER…the button works fine for a click, but I dare you to try and get it to mouse over and work on a target.
I have had all the logitech mice in the world, I have reinstalled the game, I have reinstalled Windows, it doe snot like when I hover over a name plate or a target and try to use a heal or a defensive.
I “think” even attack spells that require a target won’t work, but I typically want middle mouse button for non attack spells.
Like I can and do have it set as my Deathbringer attack, but that can be used with a hard lock target, that’s the only way, but I want you to try and use either a mouse over macro OR the default UI mouse over method and try and cast a heal on a target via mouseover without hard clicking…it won’t do it.
spells I know that don’t work right with it are Holy Prism, Barrier of Faith
I use it with Healbot, but never attempted a mouseover macro of my own. Let me see if I can figure out how that works.

Yeah, I just attempted it on Sen with the in game mouseover thingy. Seems to work just fine for my heals.
I use it for run.
It’s been run, probably since Vanilla.
I think it defaults to run.
For whatever reason, it has never occurred to me to try and use it for something else.
I had other buttons for that.
I don’t know what you mean by “without hard clicking.”
But I just used both the in game mouseover option and this macro:
#showtooltip
/cast [nomod, @mouseover] Verdant Embrace
And both seemed to work just fine when I hit the middle mouse button.
just be carful with that macro its like 75%-85% chance it will work.
My G604 works great, left and right too.
Are you using the secondary setting in key binds? Currently that middle wheel button brings up my car.

i have it set to my battle rez and i mousover click its 50/50 if the spell works an i have the charges for it
Oh weird. I’ve just been sitting here using it off cooldown on everyone in Dornogal without issue. Maybe it acts wonky in combat.
