The poisoning was the start of the rebellion which was done BY the Grimtotem. Basically they indirectly assassinated Cairne and then tried to take over.

I’m guessing he’s probably riddled with guilt over his complicity in the murder of his father, the dissolution of the Horde, the massacres perpetuated in his name, the genocide he sat patiently through, the slavery he continues to condone and the second Horde civil war he’s overseeing.
