Happened to me about 6 hours ago, I was flying from Dalaran to Broken Shore and stopped to hover for a min to chat with guild, and I didn’t have headset on and was so fixated on the chat window I didn’t notice I was in a fatigue zone until my corpse was falling from the sky.

What does it say in your combat log?
That’s happened to me while Dragonriding, more often in 10.0/10.1… hasn’t happened since 10.1.5. I’ll just lose control of my mount; fly into a cliff and disconnect. When I log back in my character is dead with a combat log that’s just says “Death” with no other reasoning (no damage taken by a source).
Again, hasn’t happened lately. Maybe you could further describe where you were and what you were doing. When Fyrakk takes starts flying toward the Suffision Camp event, he cause shadow-flame damage to any PCs that are in close proximity to his flight path with, from his current altitude down to the ground (raining fire?). I’ve taken damage while flying near him and his squad and I’ve died when he burned the ground (was funny, like when I got an achieve for being killed on the ground by Deathwing flying over back in Cata!).
Probably a sniper shot from a hunter you wronged I’m guessing.

I had a curse or something from doing a dreamsurge a few days ago. It was ticking for like 80 seconds. I was low health when I mounted and didn’t notice until I died halfway back to Valdrakken.
Well believe it or not, there are a few things that can happen in places that can still kill you mounted in the air. Be it a ticking debuff or ol dragon flying threw the zone and your in its path and it fries you.
If you were near Dragon bane keep you can get one shotted with their projectiles.

I was aggroing the UNIVERSE in tal’gurub and all the mobs killed me while I was in the air. I was jumping off the edge where the rare foe was. And died right when I jumped off
