I remember back in Gruls lair. Healer revived one of the hunters pets and the second the pet was back up it ran head long and yup attacked grul before anyone was ready for a new pull.

I was MT’ing Illidan and kept trying to figure out why I couldn’t hold threat and my abilities seemed to be missing all the time. Came to find out that my weapon was broke. Took me two pulls to realize it.

My favorite one was Alysrazor back in Cataclysm. The raid was on a break. I was tired, had a brainfart, saw the feather, and went “ooh shiny” and clicked it.
Amazingly, noone saw me do it, everyone denied clicking it, and everyone assumed it was a glitch. LOL.
It’s like the “invisible man” entry from People In Your Raid.

Waaaaay back in Vanilla WoW, back when everything was still new and shiny and we were progging Molten Core, I was playing Feral Druid (yes, i know…), and one night, as the raid was gathering and preparing buffs etc since we had killed Magmodar a previous night all the Core Hounds were gone, so I decided to stealth through and explore Molten Core. No one noticed me slinking off.
When the raid was about ready to start pulling trash, I decided to take a shortcut back to the start by… letting Golemagg smush me. And smush me he did. And then someone suddenly appeared in front of Golemagg and also got smushed. Then someone else. Then someone else. Vent started to light up with "WTF"s as Golemagg proceeded to teleport the entire raid to him one by one, smushing each one in turn, and forcing the rest to flee out of the instance before their imminent smushing.
Oh what hilarity that was… they sure didn’t see it that way, though.

The absolute best is accidentally popping Lust…on the way to the boss. Haha
On my mage, a long long time ago in the days of Naxx: I wiped the raid because I had a macro that yelled “Help! I’m an iceblock!”, which just so happened to be what DBM would make you yell if you got turned into an Iceblock during the Sapphiron fight. On that fight, when someone gets turned into an iceblock, youre supposed to hide behind them to avoid all the damage. (Think of the tree on Larodar).
Welllllll, I forgot to not use that macro, and I wasn’t going to make it to the person that the boss Iceblocked. So, naturally, I used my iceblock. Well the entire raid decided to hide behind me. Turns out, my iceblock doesnt block the raid wide AoE damage.

Not mine but during prog on the boss that drops those fire circles that cant overlap, a player ran with his circle to an existing one, while the raid leader powerlessly yelled “Noooooooooooooooooooooooooooooooo” in a very over-the-top comic book style… (except he was being serious).
Honestly,I hate wands ,please Blizzard don’t allow me one ,to shoot a boss and wipe the raid with please,pretty plaease 
They thought I would be dropping the seed before Fyrakk threw everyone off the side. Instead I dropped the seed because of Blaze. Bad communication on my part.
This other time the raid lead was explaining the council fight. I wasn’t paying attention. I was too preoccupied trying to see if I could hit all three bosses with Mind Soothe. I hit Shadow Crash instead, though.
After a wipe, I got rezzed and forgot to get out of the way of the boss, so when he respawned it got messy really fast.
Oh I got a good one.
We were progging in cata and I was not paying a bit of attention to the raid leader’s explanation (which was our gm). Mind you, we were in heroic.
I heard my name and said “pull the boss… ok” and just pulled the boss lol

The number of times I accidentally pulled Magmaw was more than zero!
Magmaw’s hitbox was huge and part of it was under the terrain so you could click a rock and pull.
Heroic Raszageth. I shifted to bear form to interrupt the adds, I didn’t cancel form properly and wound up taunting the boss.
If its the bars or the text saying how many stacks that would be big wigs. I think its emphasized warnings??? I’d have to check in game.
I’m not entirely familiar with BW as I just swapped from DBM. To many taunt timers were messed up in DBM.
If it is a WA I think its the liquid WA.

Never raided, but in old school UBRS I was sleepily tab targeting and tabbed the final boss, and sent my pet over to the final boss as we were clearing the room.
Feigned death, tried to feign innocence and was promptly called out.
They then gave me the job for then on our to kite the general out of the room for every run that followed as punishment

I don’t remember singlehandedly wiping a raid, but I did kill a huge chunk of one once. I was healing 4H in Naxx and the tank moved out of range so I had to move closer, but accidentally moved too close and chained Ziliek’s beam.
