My friend has sub every month after the every month reward and was hoping to get the every 3 months token but didnt recieve at all
If you mean the 30 days of game time, that should show up as long as your friend has paid for 15 months in total. The game time is not for the third month, but every 3 months after 12 full months of game time has gone by.
That’s how I understand it at least.

I still triggers even though you only have one friend recruited right? cause mine was gone and I didnt even get the last reward
The rewards are for any individual recruits you have. So if you recruited 4 accounts, you’d get 4 times the rewards.
Only other important note is that the veteran account (the one that did the recruiting) also has to be active (have game time on it) to receive the reward.

Yes he is active for consecutive months and when I checked the recruited friends on my acc he is gone in there.
Is it possible that your friend broke the link between your accounts? The recruit can place a ticket to ask that the link be broken if they want to. Alternatively, if you took a break for 6 months or more, that also forces the link to break.
Lastly, are you positive that you’re looking at the right veteran account? If you have multiple WoW accounts, just be sure that you’re checking the right one

From what I can see, Barouch, it has been 12 months and you have received all of the rewards, including the cosmetics and four 30 days of game time. The section below is taken from the Recruit a Friend Support Article.
Once two accounts are linked, they will stay linked until the recruit has added 12 months of paid game time.

Don’t they continue to get 30 days of game time every 3 months beyond the first 12 months? Or am I reading this section wrong?

Every 3 months after your 12 month reward, you’ll get 30 days of game time.

It’s every 3 months after your (the Recruiter) 12-month reward, meaning the structured rewards that provide the mount, pet, and other cosmetics. That specifically applies for folks who may have recruited more than one person. Their second Recruit couldn’t give them another set of the same cosmetics and mount, so instead, they receive game time.
