There is a penalty. They fail the dungeon run.

Failing the key and wasting time is penalty enough.
You have absolutely no idea how easily and badly any sort of punishment system would be abused if one existed beyond bricking your key.

Not to defend OP but you guys really don’t mind if someone just leaves your keystone for little to no reason?
This problem with people leaving Keystone is very tricky as it varies through situations.
If I’m inviting randoms to a key I’m accepting that there’s a chance I’m doing that.
The fact of the matter is simple, though: it’s not meant to be easily puggable content. If I wanted to run a key without the fear of having someone I don’t know and will probably never interact with again I’d either network with like-minded members of the M+ community doing the same keys I’m trying to do or I’d run with my guild.
So no, I don’t really have a problem with random leavers. At worst I just mutter “man, what a [insert synonym for phallus here]” to myself and try again or take a short break or do something else. But as soon as I tried pugging that key I knowingly and willingly put that option on the table.
That’s actually a very fair answer I’ve ever read.
Unless they implement a queue system for m+, they can’t.
And they show no signs of implementing a queue system for m+… so I don’t see it happening anytime soon.
Cost of doing business with pugs. Though I guess for me personally, yes I’m annoyed when it happens, but no I don’t think it needs a punishment.
Nobody leaves “for no reason”. There is always a reason. The real issue is whether YOU have the right to decide whether some other player’s “reason” is acceptable.
In my opinion, no. You aren’t the boss of me. You aren’t even the boss of Charlie. Okay, maybe Charlie…but not me!
I would love a mandatory account-wide title
“THE QUITTER”
That would let people know you are a time waster and not to bring you. You could still run with friends but you would be stuck with “THE QUITTER” till Tuesday reset.
If you are the first person to leave a m+ by hitting “leave party” or being kicked from the group after going offline you should get the title for that week.

I’ve got another idea for a title for people who fail keys all the time, and it rhymes with “The Quitter.”
Well that’s easy to avoid while still not doing anything productive in the dungeon.
With that ruleset the best you can do is sit on your hands the entire dungeon in which case you wasted your own time just as much as anyone else and precious raider IO will tell on you for incomplete keys under time or whatever so it all works out in the end.
Trolls are ok with that.
Only finished keys count.
I can only imagine people leaving for three reasons:
Trolling - but they’d probably leave as soon as it starts
Emergency - they’d probably tell you
Someone is playing badly/getting hard carried and it wasn’t listed as completion/carry
If you can’t tell which of these three it is when it happens, it’s probably the third one.
Label your groups!
Plenty of geared players don’t mind hanging out in easy content helping if they get a choice. People get upset when an easy +5 becomes a 2hr slog without warning.
Perception matters.
If I see a group of low io people doing a +2, and it’s listed as learning; I might decide to go along and give my loot to the lowest person (easy clears are a chance to chill and not use the brain for a bit)
If I join a +9 and it turns out someone is bringing a friend who dies every pull and gets out dpsd by the tank, and his friend isn’t even compensating for his lack of dps, or he didnt list it as a carry; I’m going to be upset if I take an hour and I get anima while they both get gear.
