Same. Died right at the end of the fight. Saw the cut scene while running back. When I came back to life, the boss was dead and I did not receive the spellblade.
Have not turned in the quest. Thought I’d leave it in the quest log to show that I have, in fact, completed it.
I just had the same problem. No loot when Nathanos died.
Same, died before the cutscene and when i rezed, there was no loot or wq. i was able to turn in the quest.
i went and killed him again, but no wq or loot.
I had this issue yesterday. I took part in the fight and died (which isn’t unusual) but the fight finished while I was ghosting back to my body. I got no reward and the fight icon on the map was still there, even though I’d participated in 98% of the fight.
I had to do it again (making sure I kept myself alive, which is hardly in the spirit of group content) to make sure it didn’t bug out on me again.

Just like with any wq, you have to be in range to get credit.
The cutscene sure as hell thought I was close enough. It ran long enough for him to respawn, no body to loot. It will take like… 10 minutes to get in on another kill. Ridiculous ;p
EDIT: It’s about 180 yds for the world quest loot, graveyard is over 450yds away
So far have run over 20 alts through this WQ. I found I don’t get credit for the kill, which happens if Nath drops below 10% health, if I’m still outside the quest area when running back from graveyard. If I’m within range, get the weapon even if I’m still in ghost form and haven’t rezzed.
True, the reason I stated below 40% is due to the run time, after he dips below 40% there is a very high chance that you will not make it back in time to be in range to activate the wq.
so i tried last week twice to get Nathanos weapon on my pally no luck but got cut scene both times. killed him this morning an i died same as last week but got a weapon.
