Seems to be a new bug in Korthia. The Spectral Bound Chest appears (with the usual chains on it). Once getting all 3 keys, the chest actually completely respawns and is not able to be looted.
I’ve tested this on multiple characters between Illidan-US and Tichondrius-US

This happened to me too, the chest despawned as soon as i looted the third key
Just to give another update, I wanted to find a way to see if it can be resolved. Leaving to The Maw and going back doesn’t spawn it. Going back to my Covenant Hall and then back to Korthia spawned it.
I am assuming by leaving the zone area (probably even Oribos would work), this seems to respawn it. I’ll test it on another character and update.
It actually just respawns after a few minutes. I’ve had to wait at it’s location and it eventually just shows back up. You don’t have to leave or do anything else. It’s rather annoying and makes the daily chore take even longer, so I’ll probably just skip it from now on until it’s fixed.
I just relogged and it showed up.
Relogging worked, but they need to FIX it.
The last 2 days I havent had any issues. I assumed they fixed it but I guess not?
Noticed this the few couple of days but then everything worked fine. With the issue the chest would vanish once I picked the third key. Very odd but it always came back after a few minutes.
I had to relog today. It’s done it every day since 9.1.5.
Every time I get all 3 keys this chest despawns. The first few times I just walked away, but it does come back after about 5 minutes. Really wish they would fix this.

I also waited as I was looking up the bug report and lo and behold, it respawned … with all the adds … so annoying.

I normally don’t have this issue on my alts since patch, but I did on my main earlier today. I tried logging out but it didn’t help. It eventually respawned.
Very irritating and hopefully the addition of a loot timer to this treasure object was not an intended change.
This is still happening randomly for all my characters.
The random times the chest disappears I run over to whatever rift portal is up and it’s in the same spot just inside the rift. Seems like one day it’s outside, next day it’s riftside, then next day outside, etc.
but yeah if the chest disappears for you try checking the same spot but in the rift.
Has happened to me several times. Just happened to me tonight. Happens enough times it almost doesn’t feel worth it to track down the keys… please fix this Blizz.
As stated above, it did respawn, but seriously… it is silly lol
Super annoying just sitting here waiting.
Tried hearthing back to my sanctum and coming all the way back out… Didn’t respawn. Have killed the mobs twice now around it… I just want my dang loot.
This happened to me about a week ago (right after Tuesday reset of course) and I tried leaving to finish up Korthian quests, get nests, rares, kill the different bosses that showed up, but each time I came back I could see the chest but as I got near it despawned.
So leaving the area entirely was the only solution I could find to fix this issue. I even tried logging out of the game and back in before I flew to Oribos, emptied my bags in the bank and then flew back, only to find the chest available again.
It seems that with each patch or reset a new series of bugs are added to the game.
