I’ve a new lvl26 monk (not in charater list here) when underwater there is no breath bar. Using the edit screen I’m not noticing any different choices from this character who does have a breath bar when underwater. I’m missing something, but what?
Undead have underwater breathing as a racial ability (it’s part of Touch of the Grave). And some NPCs who give you underwater quests will automatically give you an underwater breathing buff at the same time.

The character in question is a BE monk and drowned on the Seth’s Graphite Fishing Rod quest when no breath bar was present and I figured perhaps underwater breathing was now normal…
OK, that’s odd.
In Edit Mode, the Breath Bar would be under
Advanced Options → Misc → Duration Bars.
Ofc, it could also be an addon. Try with all addons disabled to check that.

Hm. I wouldn’ta thunk it, but apparently people have had this problem. I never heard it before.

Interesting. I had played with Edit and found the duration setting. In Edit mode three bars appear but none of them are there when the character in question goes underwater. Further experimentation showed it did appear on this character and another character but did not show on a lvl 2 BE paladin who is one of my banker characters and has existed for 15+ years. More character checking will be necessary
Edit, added two days later:
Interesting, tonight the BE monk had a breath bar and I’m not aware I changed anything since the previous underwater adventure without one being present.
