When fighting Smolderon i get this error and after a /reload everything is gone, all addons disabled an most spell icons are missing.
The error is:
load lib error
Since this is a UI issue, you’ll want to ask in UI and Macro.
A UI reset is probably in order.
Definitely doesn’t sound addon related given what they described in the UI forums.
RAM, drivers, or something hardware sounds much more likely.
Seems to only happens on WoW specifically.

== FOUND THE ISSUE ===
The issue is related to TTS.
If you have a custom Text to Speech that you removed from Windows and ANY addon try to speak with that voice, that error will popup.
