In my run today at 11:10 PM Mountain Standard time, we were attempting to clear Halls of Valor. It was going well, until the boss Hyrja somehow got reset to her original position after coming down upon the defeating of Solsten and Olmyr.
We noted that our resto druid’s adaptive swarm was jumping from us to her and affecting her health, which was rapidly regenerating. This didn’t cause aggro, and she never attacked us. After it jumped several times, she suddenly warped back up to her original platform, and we were unable to attack her or get her back on the ground to initiate the fight because of this. So the M+ 5 key was effectively impossible to complete.

I had this same problem tonight. Did the two minibosses and boss went back up to her platform (and thus out of line of sight) after her RP but before she became attackable. Adaptive Swarm jumped to her (though I didn’t see it affect her health) before she warped back as well so that’s probably the cause.

I was in Halukari’s group. Seems to be a fairly common occurence: Halls of Valor Mythic Plus - Hyrja
Happened in a group just now, adaptive swarm as well was bouncing around on her not causing any aggro but she ended up jumping back on the platform.
Group tried a wipe, instance in & out, and aoe to see if there was any way to get to her however unable to and burned the key.
This happened in a +15 Halls of Valor today - appeared to be triggered by Adaptive Swarm jumping to the boss while out of combat. There was no way to reset her - tried clearing to Fenryr and coming back, and tried fully zoning out and nothing worked.
This was reported 9 days ago and is actively bricking keys still with an ability that bounces uncontrollably- how did this not get resolved yet?
Just adding my voice to the others here, same thing - Adaptive Swarm touched her out of combat, causing her to teleport back to her little RP stage behind the room. There was no line of sight to pull her back down, and the key was lost.
Hoping for a fix soon.

This happened for us as well - a 15 Halls of Valor got bricked thanks to Adaptive Swarm sending Hyrja running scared back to her platform xD. Awful
Just happened in my key… a month later, I guess this bug will never get fixed lol
Wasn’t sure what caused it until I read this thread and realized I am, in fact, the druid using adaptive swarm 
This just happened to me as of last night so it’s still happening. Completely bricked a +13 so feels great.
This is still a thing… bricked my +20 key an hour before reset. Absolute ridiculous.
I encountered this bug in a +18, we reset her by having someone with spiteful storm target her and start casting at other mobs we pulled near her, the storm bolt reset her. Hope this helps someone
Other comments said that Potion of Shocking Disclosure could fix this issue, but we had not success with it. Also targeting with a ranged trinket supposedly works, but we got it to work with spiteful storm.
