It’s rng unless something has changed. I did it in legion while watching netflix. Believe it took about 2 hours. That said, I know others who’ve gotten it more recently, so it’s still available. Only thing not available is the mage tower. All other skins are still obtainable.

Not sure, but I would guess you need it equipped for the item to drop, but that’s purely a guess
