Waited over 5 minutes before getting her to 65%
Killed all mobs
Waited 5 minutes during the tidal waves before killing any tentacles
Saved 1 tentacle for after the tidal waves
Tried many different things
Still won’t prog to phase 3
I’ve been in this mythic raid for almost 3 hours now trying different things.
After a few weeks of following all tips and trying different difficulties, I finally had luck with Heroic 10-man, and on the same day of a bugged 25n. Good luck to you. 
It happen to me, I then push her to 65 and stopped dps and the phase worked like normal she did come back.
Confirmed that too - don’t dps too hard on the first phase, let Heyla summon waves 3 times, kill a few tentacles between wave sets, take your time and don’t rush it. And phase 3 will activate.
If you’re a class that can use ranged, have it on standby to whittle her down from range… Maybe?
I had this issue with my War. Cleared trash, and she bugged right into phase 3 after killing only 6 of the tentacles.
Never had any issues with my Hunter blasting her from the shore. But I can’t remember if she goes full immune during Phase 2. So this might be absolutely worthless and a waste of your finite seconds on this world. Apologies for that if that’s the case.
Still happening. Not sure why we still bother reporting bugs on here in old content since they never fix anything.
**And yes, I tried some of the fixes above several times. Sometimes she will just never enter the next phase. I reported this same bug in game like a year ago and never got a response.
Can confirm that this is STILL broken, to this day. For crying out loud Blizzard, WHAT DOES IT TAKE

Two years later and I’m still having this issue with Helya
Two years later and I’m still getting dismounted flying over the chain in Zereth Mortis.
These are things we’ve reported AND YOU’VE DONE NOTHING. I don’t care that it’s legacy, that shouldn’t make it something you ignore.
Remember when Blizzard was quality and Bethesda was the buggy one?
Now you both are competing for buggiest gaming company.

No it’s not a mechanic, because she’s supposed to transition after the tentacles are dead.
there are requirements other than just “me smash, u die”.
it’s a fight with phases.
if you break the progression of those phases by obliterating everything, you break the script.
