Just experienced this as well.
I had the same issue. Try to kill the last tentacle while she is NOT channeling her waves, that worked for me.

I was having the same issue on Mythic.
This worked for me.
Confirmed. I am stuck in the instance right now in normal mode then she will not transition to phase 3
killed all adds in zone, still no phase 3, waited 30s same, killed helya to 90% waited for the slime adds to explode then killed to phase 2, immediately killed the 3 tentacles at the back then killed adds then other tentacles then more adds killed and Heyla finally went to phase 3

Tried four or five times before getting her to transition today. Cleared all mobs, no luck, waited during phase 1, no luck. Then I tried waiting until she was done spamming tidal waves to make sure I killed the last tentacle outside of that behavior BUT immediately after she finished the tidal waves she transitioned to phase 3 while that last tentacle was still alive at full health, then the tentacle just disappeared. I was confused but went ahead and took my W.

Worked like a charm set a stopwatch threw a knife at her at chilled for about 1min burned her down killed tentacles phase 3 killed her got the win thank you
Helya was stuck on Phase II for me also. after reading these replies, I decided to let the waves kill me and start the boss sequence again. This time I played slower, to allow enemy groups to spawn naturally, then Helya progressed to Phase III.
Still bugged as of December 29.

Kill the last tentacle while she is not casting the wave ability, will transition. Couldnt figure out why my 70 mains had no issue but my 60 did until I did that.

Got it. Had same issue smoking all tentacles quickly she wouldn’t move to phase 3. Killed all but last tentacle and waited, she came out of it and died to imps.

I solved this just like this:
- Attacks Helya for 2 stage
- Destroy 8 tentacles and save 1
- Awaits 30 seconds and… here we go.
Tested 3 times and it worked for me!

I’m doing this quest on a different toon that’s horde and after destroying all of her tentacles she does not return to land. she just keeps sending heaps and heaps of trash mobs and nothing happens if I kill them before the meter is full.

This has been very annoying for me as I’ve had issues with her not transitioning to P3 for a while like about 6 months now. Every once in a while I can get the transition but have not really consistent work. Really though it is bugged and should get fixed.
Same here worked one on toon then bugs on all others.
Been checking this out and as far as I can tell the main thing is to make sure you let her cast Blighted Breath and spawn the oozes before you proceed. After that downing the tentacles and everything works as fine.
Having the same issue. Despite only using basic attack so she goes through all of her motions, she is stuck in phase 2 (waited 15 mins/ 6 rotations after the tentacles).
I’m trying to go through older class content for the mount and this has me stuck.
