I’m a senior with artritus and carpel tunnel. My hands just can’t do the right and left click like is use too. I purchased a USB pedal device to help with this. It requires “Elfkey” software to keybind pedals to keystrokes. Is this sofware a violation of the TOS?

Anything beyond mouse and keyboards can toe the line of automation (even though this isn’t a case of multiboxing), which is a big no-no.
I would suggest you shoot over an email to the Accessibility team. While responses are not guaranteed, a lot of folks we’ve sent to them have managed to be answered. Perhaps they can advise you in ways to facilitate your gameplay without putting your account at risk.

Please update with what you hear. A foot pedal is no different than a mouse. I know someone with severe hand complications that uses one instead of a mouse. I’m very interested to hear what they have to say.
I think a foot pedal should fall in the realms of regular hardware usage, provided no automation or other gimmicks is going on in the interests of accessability, as Kaleberry said do reply back with what they said to you.
Without the software, the three pedals type A, B and then C. I could use it this way, but it would be much better to change keybinds
Heres the tricky part each keybind can only do one action not 3 different things at once.

I still think it be worth looking at doing what Leilleath posted, just to be safe, Nayaga. After all, don’t want to do something then find your account is at risk of suspension because of something along the lines of automation when it isn’t.

The other hardware I’ve used is the 3m vertical mouse. No wrist action, thumb action instead of two fingers for mouse.
Looks like a joystick, but that’s not how it works. Also, no software required, works like a regular usb mouse.

I use footpedals (well, rudder pedals) – have for years.
I don’t use Elfkeys.
It’s a pretty touchy subject. There are certain pieces of software on a blacklist, for the rest I expect it comes down to what it does and what Warden detects it doing.
Don’t forget you can keybind the pedals with modifiers – e.g. alt+pedal1.
I’m assuming that if I don’t use software, I should be ok. Hardware is just a keyboard that types the letters A, B and C
I am sorry Thwapsome but the ADA laws regarding disability access do not apply to video game entertainment. The only part that is currently governed by law in the US is communications. Those have to be ADA compliant. The rest does not apply the way access is mandated for government buildings, essential services, etc.
Blizzard does a LOT to bring games to as wide an audience as possible, and has a whole Accessibility team.
What they won’t do, is allow persons to automate gameplay to cheat, regardless of their ability, or disability.
In the case of the OP I can’t comment on specific software, but key binding the pedals so that one pedal push = one in-game action is fine. What they don’t allow is anything that automates portions of gameplay - as in one press = multiple actions at once. Any macro type stuff should use only the in-game macro maker.
People have been using basic key mapping for foot pedals, hand controllers, and other devices for years with no issues. Stick to the one keypress = one in-game action and you should be fine.
Beware they will probably tell you that - and also tell you that they won’t approve/reject any individual software specifically. If Blizz does not make and control that software they really can’t give blanket approval.

Foot pedals are fantastic. Friends use them for work tasking, gaming, etc. It is just another input key recognized by the computer. Like, I could bind rogue kick to a foot pedal - which would be cool. Single key press - single action.
Arthritis, injuries, differently configured hands, all those things make foot pedals and adaptive hand controllers a must.
Hopefully you can set up your foot pedals to give you additional keybinds that are helpful!
Mirasol is extremely knowledgeable about the accessibility portions of the game and the TOS/EULA; I would absolutely trust them on this.
I’ve been using a pedal for years. I don’t use “elfkeys”. I have a software to change the keybind, but it doesn’t need to run all the time.
