Patience is the name of the game in gearing up doing world content. Keep doing Ivus and the darkshore warfront quest and the 395/400 emissaries and you should reach 400. I have one character ilvl 404 just doing that but it took a long long time.
That endgame is coming in 8.2 for those people.
