This is still happening to me. On all my toons and it’s completely random. The portrait will come back on its own sometimes and then disappear again. On and off. Really annoying especially for pet classes cause the pet’s bar is gone too.
Important to note that this is with the Blizzard UI, not any custom UI.

Glad to see it’s not just me - I was trying to figured out what addons could be causing it, but sounds like it’s a ZM specific interface issue.
I’m now having the same issue. I’ve renamed cache and two other folders and that seems to help for a little while…then poof gone again. What is going on?
I JUST had it happen to me for the first time. I killed an NPC on a world quest in ZM and poof. Leaving the game and returning, did bring it back. Definitely needs fixing asap.
UPDATE: ALWAYS happens when taking damage in Endless Sands. It happens elsewhere too, not always when taking damage. But in Endless Sands, when I take damage, I lose my portrait. Can’t see how much damage I’ve taken and have to rely on screen-flash and prayer to keep myself alive.
Has happened to me 5 or so times now. /reload seems to fix it for me, but kinda annoying - especially if you’re mid battle

Glad to know I am not only one this is happening to and like others it started after I got to the cache part and puzzles seems everytime I do a puzzle then go back it is gone hopefully this will be fixed soon as it is annoying.
Yup. It has happened repeatedly the past week. Multiple toons. Always comes back with a simple “/console reloadui”. But annoying as all hell.
It happens all the time every 10 mins or so I have to reload for it to come back, I guess this will be presented as a feature on next expansion
and nobody from blizzard has yet responded ??? This is frickin annoying. This issue needs to be address ASAP !!!

This keeps happening to me so I did a UI reset. It’s still happening and I don’t have any addons installed. yes, a reload fixes it, but it happens to me in combat and it’s harder to track my own health without it. I have friendly health bars turned on so at least there’s a bar that I can see without reloading during battle, but that bar just doesn’t work as well for me.
This has been happening to me repeatedly, and just happened right now to my husband in the middle of combat. This makes the game pretty unplayable.
put /reload in a macro and put it where it can be easily to be clicked on in battleor push F1 to select your self
i know it’s not a fix but it’s a workaround until Blizzard responds or undo whatever they did in the update and gets around to fix it
Just happened to me again 
Continues to happen in ZM, if there’s a popular post on reddit I’m sure it’ll get fixed, until then I think we just have to continue to /reload, which btw people you don’t have to type out /console or even UI, just /reload fixes it. <3

This happens multiple times to ALL of my characters in ZM… every day!!
I hope they get it fixed asap. Sometimes I’m in the middle of a fight on a named before I notice that the frame has disappeared.
Plus on my hunter, the personal health bar gives me zero info on the pet’s health, so it makes it hard to know when to heal him.
I have logged in 3 characters so far today in ZM and NONE of them have lost the avatar frame. So it looks like, unless something changes, they have fixed the problem. fingers crossed

How is this still going on? It only happens to me whenver I do one of the NEW cache puzzles. The two puzzles that are copied from Nazjatar (Bejeweled and the ball line link one) don’t cause my character frame to disappear. It certainly seems like whatever code they have in place to hide our portrait in the puzzles doesn’t get turned off when you finish it, unlike the two older puzzles that were (presumably) copied over.

I noticed that it seems to be fixed today as well. Did the WQ for the cache puzzle and my avatar did not disappear.
