That’s good because achievements also didn’t exist in vanilla.
Imagine playing a game for over 15 years and you haven’t done anything good in the game. Not one good achievement that shows you at least have a little bit of skill.

they didn’t change the cave in tbc because alliance always won

Actually they did change it in AV after reinforcements were added and horde started playing scorched earth. And just like now horde had a near 100% win rate.
let me assure you there were no reinforcements in tbc

Reinforcement mechanic was added in path 2.3 and then a balancing patch in 2.4 that moved the cave back.
