Other warlock in my guild is rocking 40 IL titanforged pants from normal. You know…that non-azerite slot with the most stats on it? He’s never replacing those unless the mythic ones titanforge. THAT is the issue with the system.
In this case the stats were not ideal. In fact, I had to do a very detailed comparison to see if this 60 ilvl increase over the item it replaced was actually an upgrade, which is a whole 'nother can of worms about itemization.
And add to that, that the ilvl upgrade actually reduces my effective base damage in PVP due to ilvl scaling, with the item being a possible upgrade only because of the proc having more uptime than the previous on use item, and it’s a “feels bad” situation all around.
You cannot hide your armory, learn to use the website. This is a you issue.

There are supposedly a couple of a million people playing this game and many have a bunch of alts at 120. I have 12 and I’m not a dedicated altoholic by any means.
So, lets say there’s a minimum of 2 million toons being played at 120, if something has a 1:100,000 chance of happening say - twice a week - then it’s likely to happen to ~160 toons a month.
So being rare does not make it a bug any more than winning a lottery is a bug, and how you feel about it does not alter the fact that wild titan-forges are programmed to very occasionally happen. They are rare enough that I’ve never heard of anyone getting more than one titan-forge of over 15 levels, and never personally known anyone who has got even one.
The idea that people are gearing to mythic level through super-titan-forged quest rewards is laughable.
We have no idea if there are 2 million or 10 thousand playing anymore since Blizzard is too ashamed to release numbers anymore. Please stop making stuff up.

ASSUMING it’s the correct Irisse, “on Maelstroms.” They have ZERO 420 pieces of gear. They DO have a 415 titanforge, but it’s Kaldorie Archer’s Greaves which came from Darkshore’s quest so that’s only 15 levels.

How do you know it was once in a thousand? The drop rate isn’t determined just by your own drops. It takes into account everything region wide. The chances of getting a +60 would be extremely low.

Hey look at that, they’re on the armory because you cannot hide a character at level 10+ from the armory.
