Lunara, Zagara.
Imperius heals himself too much, he has an area stun and if that wasn’t enough he can make the boss himself from lvl 10.
But of course he is a bruiser and nobody complains about bruisers when they are ops.
As for counters I think Varyan smash can be a good option, also li ming orbs build.
In Braxis rexxar should work.
Otherwise, mots ranged assassins will.
Lunara especially. But Fenix, Gul Dan, Raynor etc.
He only heals if he hits you after branding you.

A hard lane pusher with pokes and some kind of escape. I’d say Gazlowe so he can afford to go close enough to catch the xp globes if Imperius freezes his lane. Goblin repair and Robo-Goblin.
Where do you want to counter him? On lane, long ranged, easy to kite with Heroes counter him.
Sololaners with better pve (camp taking, like Malthael) should also work.
In fights, kite still works, and CC to interrupt his Q. Also, blinds are annoying as him.
It is also funny to see him struggle against an Illidan. Directly in lane, pretty much any ranged hero, or one who can prevent him from healing off his trait reliably works well. Also note that his PvE is below average, so sending camps down his lane will keep him on his back foot, too.
I can hold my own against one in lane on Artanis, but I have to stay on top of my game. If you do play a melee hero into him, focus more on dodging his Q, forcing him to choose between hitting you or your wave with his W (ie, don’t stand in your minions when his W is available), and when he uses E, move away or move into your wave to reduce the number of ticks/brands he can get on you with it.

though actually i have had good luck with sylvanas. i mean if you keep moving around it definitely makes it harder for him to stab you.
if i were needing to just out right counter him in lane i would probably pick up ray because his range is enough to keep him mostly safe. plus he has a heal and knockback in his kit which helps.
There are general imperius counter-measures, and then there’s build-specific ones.
Imperius can tend to build around 3 ways; Q impale chains, W poke, an AA duelist. Q build wants to kit several targets in a line, so a decent way of undercutting that is either to not pile up, esp in narrow choke points or to have some cc/displacement ready to interrupt his Celestial Charge; losing his combo tool to a cooldown is a good way of keeping him down if he builds that way.
W has a bit of variance to it, and pretty much tries to play it safer with having some ranged poke/refund before aligning a combo. Both Q and W are linear and heavily conveyed abilities, so, funnily enough, some melee and mobile heroes can just enter into Imperius effectual blind spot as he’ll have a harder time landing abilities to get marks, to then consume marks to deal damage and sustain himself in fights.
So Q one would want to avoid grouping up and keep distance, W can try rushing him.
AA/E Imperius is different in that he can trigger lots of marks from molten armor, gets aoe attack swings and offsets some of the weaknesses of the other builds (not as reliant on the full combo, potentially better waveclear) but it takes a higher level for that build to pan out. In an AA build, swarming him could be a bad time as he has better aa/talents for trading hits and isn’t compelled to land his combo, at which point, getting in closer to him does him more favors than not.
So that’s sort of the build-based concerns, and a decent chunk of those can vary based on his lvl 1 talents – assuming you want to revise strategy once the match starts – but there are some generic concerns as mentioned in other posts.
He’s pve control isn’t very good so heroes that can out push him and out range him and/or interrupt his combo attempts can keep imperius in check early/mid game, esp if he’s the solo lane.
However, he can also slot as the ‘main tank’ role and that team then have a better/different solo laner that then also brings follow-up or enabling cc for Imperius. That can be a bit more problematic to deal with as Imperius offers a lot of threat/pressure until he’s committed to using his combo.
General rule though is that he does suffer from poke (he has in-battle healing) and still wants to place and consume marks, so isolating him, and then bunching in upon imperius can offset his ability to land Q’s – a lot of players don’t have good practice hitting up close targets, and can even over shoot the intended impale thus burning out a key part of his threat.
Specific hero wise:
Trall has poke and in-battle sustain to rival imperius plus he has double cc (root and stun with heroic) to cripple, stop, or just delay an imperius impale (he can’t use it while rooted)
Sonya beats him on pve, can probably trade hits with him in a dual (I haven’t tried that personally, probably build dependent) and she has two stuns to offset/interrupt his combo.
Xul/Rag a bad charge-in can get either of them impaled, but otherwise they can out pve Imperius, and if Imperius isn’t the solo front-liner, than their talents for getting healing & cdr from melee targets can get them to out last Imperius in trades.
Malf/Anduin less so in a solo lane, but just in general, their ability to root imperius can really give him a bad day if they preempt a root on/near an ally about to get Impaled.
Arthas: more of a theory craft based on what I’ve seen on other match-ups, and also based on what I wrote above, but Arthas does have root options, can get in close in imperius, can debuff him and, build dependent either out-poke imperius or out-melee him, but again, some of that depends on either heroes build. Arthas does also have a better pve game compared to imperius, but Arthas is more a team-based contributor rather than a solo laner (like sonya)

He has weak waveclear.
He is Q landing dependant even more if he goes Q build. If he misses one he is an easy kill.

Well blinds also screws W xul very hard especially if you land one the moment he activates it preventing him to ramp up his cdr.
Nova can be a good counter if she atacks him from afar while staying out of the range of Imperius’ abilities; her slowdown may help her stay away.
Except Cassia. She is very vulnerable to Xul’s rooting.
True if it is a TF and cassia gets rooted and jumped on with her trait down.
In that case she has to rely on her team counterdiving/ cleansing her.
If it is a mano a mano cassia can poke him from afar and if she gets rooted she just has to wait for him to get close and activate W to use blind and ruin him.
Xul if he goes W is weaker in 1v1 situations fightning him head on till 16 is not a bad plan Imho.
Against my Imperius? None.
Against others, I might give you a few tips.
So far I haven’t seen anyone list Imperius’s biggest weakness.
