Played against a priest (shadow mode) who had 3 cards in hand, empty deck. This person played Zephrys the Great, wished a card, then did 2 damage to it killing it. Allowed this person to repeat the card over and over, until I fatigued out. Each new turn their hand count went up by 1 even though they were fatigued. Was that a hack? There’s nothing on the card that puts it back in hand, unless there’s a card in their hand that does this. Am i missing something?

You might have encountered a bug which has been showcased by HysteriA (and most likely many others). Using that bug against another player would most definitely be an exploit. I am not linking to the videos showing that, out of concern that showcasing it is showing how to exploit it, and that would be against the ToS.
