Occasionally Sightless Watcher does not place the requested card at the top of your deck, has anyone else experienced this?
was a bomb or a shard shuffled into the deck before you drew the next card after playing sightless?

Hmmm, I believe so, a shard (soul fragment). It has only happened twice so far.
Then that’s not a bug.
Shuffling cards into your deck shuffles your deck. Imagine that…
