In the 28.4.1 Patch notes, one of the bug fixes reads:
[Hearthstone] Fixed a bug where Shadestone Skulker could cause infinite loops. Now, when the weapon is “given back,” the Skulker will no longer have it—meaning it will only return a weapon the first time the deathrattle is activated on a particular Skulker.
yet I just played a game in Wild where Shadestone Skulker returned a Necrium Blade, then stacked another Necrium Blade on top of it due to Snowfallen Graveyard, breaking the first one. I would’ve been annoyed losing to this combo, but would’ve understood how it worked. However, according to the patch notes, I SHOULDN’T have faced that exact interaction.

are you saying that it eqiuped the weapon then another and another, and another, and so on, forever
or did it equip it twice?
It equipped the weapon twice. Am I misunderstanding how they fixed the issue, though? It says

Perhaps I’m misinterpreting it, but to me that makes it sound like the deathrattle goes off, then the Skulker no longer has the weapon. So, no matter how many times the deathrattle goes off after the first, it shouldn’t reequip the weapon. What I’m guessing they ACTUALLY did is program it so the deathrattle tracks the weapon until all instances resolved, then clears the weapon data. So, if you have something that makes it recur immediately, it still works, but if something triggers it outside itself (i.e. Xyrella, the Devout, Play Dead, etc.), only the FIRST instance will return the weapon, any other instance (its own deathrattle or another trigger card) won’t do anything.
Edit: thought of an example (Well, two of the same example): The old Malorne, and the Solo Adventure treasure Elder Taggawag. Both say “Deathrattle: Shuffle this into your deck”. Even if the deathrattle goes off multiple times, only one copy is shuffled in. Also, Doomerang checks if you have a weapon equipped between casts, so if you have a way to double cast it (got the Shaman quest reward Bru’kan somehow), it only throws the weapon once.
there was a issue before where it would go on forever, and you would have to disconnect then reconnect to keep playing, but by the time to you get back its either the combo won or your turn is about to end
it was a crazy thing to watch, there should be a few videos of it on youtube
