When using the hero Fire Mage Jaina and playing the “Ancient Mysteries” card (draw a secret from your deck it cost 0) the spell does not work and no card is drawn. The card works sometimes but more often it does not work at all. This is happening when using the standard spell mage deck.
Sounds like there weren’t any secrets remaining in the deck.

Plenty of secrets in the deck. This deck is built on spells and secrets and it happens in the beginning of the game as well before any secrets have even been played.
did your opponent have counter spell?
Do you have a game replay or video recording of such? Or, since it happens a lot, can you make a recording of the next one?
