It is my understanding (right or wrong) from comments whene you google that if you run sanctum you will get 4 keys per day.
This is not true. I have never received 0 but very frequently I do not get 4. Today was only 1 on one toon and the other got 3. This happen too many times to count. Occasionally there is some monster stuck in a wall that you can not damage and maybe it is holding 1 key. This still not explain why such a wide result of 1 to 4.
Perhaps my understanding is wrong but I think that someone from Diablo should clarify.
If is truly suppose to be 4 as I have seen commented, then Diablo should make it easier on themselves. Rather than try to put 4 keys across all the monsters maybe opening the door to the first room should give the 4.

You will get up to 4 keys per day, dropped from mobs you kill inside the Sanctum. The number obtained is variable. However, I’ve never obtained less than 3 so your result of 1 seems highly unusual. Bear in mind that if you run off from where they drop, the ones you missed picking up whilst inside there will arrive in your in-game email afterwards.
I realize that you have to pick them up. Many times due to my concern I have travelled back frequently during my run to check. The only 2 or 3 runs that netted just 1 there were guys stuck in walls where I could see them but not kill them. I have has many 2s and even more 3s.
I am well aware of this issue. That would be a dev’s mistake.
Temporary solution: Check the number of keys, and if it isn’t four, do not open the first room door, but exit Iben fahd and enter again. You will get the remaining keys on the second try. If you can’t get them on the second try, try it a third time.(never open the door)
