Could it be that you had sent your familiars to do the Sanctum before doing the run? They pick all keys and the rewards from the first chest, so you can’t get them if you do the Sanctum afterwards.

I imagine it’s because you sent out your familiar(s) on an Iben Fahd mission…

…and you went into Iben Fahd’s Sanctum prior to the familar(s) completing their mission.

The familiar mission gets the four keys you’d normally get from killing the mobs prior to reaching the first room and the Sapphires, Garnets and Beryls you’d get from the first chest.

Thus, if you enter the Sanctum before the familiar mission is finished, you don’t get the 4 keys clearing the mobs and you can’t open the first chest, because if you could, you’d get double the keys and gems.

This is NOT a bug.
