Here are a couple of the main threads on this issue:

For a few players having this issue, there has been some correlation between system performance and timing between teleport initiation and completion.
Its almost like the game has a shorter “wait” window for checking for internet during a teleport than it used to. And if your system has anything hindering the performance too much (IE, slow drive, memory issues) it may timeout, blackscreen and give you a disconnected error.
But this is purely speculation and not everyone may fall into this potential issue.
Not to mention it isn’t affecting everyone, but a few of you. However enough to warrant being looked into by the team and see if they can find where they goofed up with the latest patches.
Till then, not much else can be done except to go over your setup and make sure your system performance is at its peak:
- Shut down programs you don’t need.
- Shut down any potential screen overlays or other similar apps.
- Don’t run web browsers in the background.
- Shut down the Battlenet app.
- Don’t run streaming services to play music or video in the background.
- If streaming, temporarily suspend streaming activity to eliminate that.
- Programs like Geforce Experience and AMD Adrenaline cause issues too.
- Check your drivers, updates and settings.
- Make sure your hardware is running at comfortable temps and not being stressed to max. (lower settings if need be)
- Make sure the game is running from the fastest drive in your system, preferably a decent NVME SSD.
- Check your memory profile settings. Sometimes things like XMP can actually cause issues on some setups.
- Remove any overclocking settings to eliminate those as an issue.
Mostly use some common sense and think it through. Good luck.
