Even though Blizzard has always maintained that “There is no cow level”, players have discovered such secret level in Diablo II.
When Diablo III hit the shelves, there was indeed no cow level, but we got Whimsydale and Whimsyshire instead. Replacing our bovine friends with rainbows, unicorns and teddy bears.
Once more Blizzard has stated that “There is no cow level at launch in Diablo IV”. This doesn’t deter players from banding together though and there have been quite a few promising discoveries.
The Book of Lorath makes references to Wirt’s Leg and Bell once more, these were needed in Diablo II and III respectively to open a gateway to the secret level.
There have also been 4 data mined items with an attribute “Quest_World_SecretCellar” . All 4 seem to be very promising leads, but there’s a huge problem. No one has a clue how to obtain any of them.
Y’all need to stop lol Rob confirmed (Google it) that there currently is no Cow-Level, but said that it doesn’t mean there won’t be one in the future.

On being asked about the prospect of a cow level, they said “At launch, Diablo 4 will have a gritty and grounded world. A cow level, wings, or pets take away from that groundedness”. With the developers aiming for a darker tone for the game, the cow level looks to be one of the casualties.
While there won’t be a cow level from the start, the door is open for it to make a return in the future. Shely and Fergusson noted that “things will loosen up” as the game’s development continues, and less serious content like the cow level could definitely make a comeback, especially with its popularity with fans.
