From what they have revealed and the actions they took to “fix” the bug.
They just have spaghetti coded loot tables. They had ubers on the same tables as regular uniques which was poor design to begin with.
This could have been completely avoided if they had a better design initially. The fact that they had to just make uniques not target farmable in helltide chests going forward shows that the spaghetti runs too deep for them to do it properly now.
This wasn’t from a typo or anything in their code. The items were dropping because they overlooked the fact that if you hit the unique drop table on a slot your class didn’t have a basic unique for, it forced an uber unique.

Shako would be nice to help explore alot builds, help finish the shallow parts they call end game, instead of grinding out .5-1% dam reduce upgrades on my pants.
They didn’t actually have a fix. They had to disable uber uniques then completely change their initial plan of having them be slot targetable via the chests and just have a chance to roll any unique regardless of chest type.
They had to do a workaround and change their plan because of their spaghetti code. They did not actually fix it.
They would need to re do their loot tables structure to fix it properly.
