Belial need to ambush me every other time I kill a boss. He seldom drop 750s. When he first ambushed me, I yelped. Now it’s more like ‘hey buddy’.

I’ve never gotten a mythical from him (Baby Belial)… Only the Boss of that particular arena.
@ OP , are you by yourself or with other people.
I’d imagine the odds of Baby Belial appearing increases if you have more people (Especially if they have their ashes on baby Belial appearing…) 
When he ambush me, I was playing solo. I did group once, but he didn’t show up. Oh no no no…i’m on the Eternal realm. (just caught that). Yeah I don’t play seasonal anymore. I would guess ashes give a boost to him showing? I don’t have a booster unfortunately.
It’s really Random. Diablo 4 is about 88% RNG. You just have to keep at it. I do recommend you try groups though.
I’m also sure Baby belial doesn’t show up on his Own fight…not sure why there is a TIMER?
They need to Fix that Belial Chest on the Belial fight. People open up the chest at the same time and they can’t, they just run off in the lower left hand direction because Somebody Else opened up the chest before them.
