Using a healing well while dying softlocks the game

Single player, adventure mode, happened in Alcarnus. Used a healing well as my character died, which left the game in a softlocked state where my character is clearly dead, but the revive options never appear.

Only option is to quit out to the menu and start a new game session.

6. Can’t revive in a rift. Happens in both Normal and GRs. This is very random and the only work around is to make a new game.

This normally happens in rifts when you level up right as you die. Can also happen in the open world, but people don’t encounter it very often since they do not spend much time leveling up and dying in the open world.
