Seasonal : Barbarian skill Nerves of steel, does not work. Died 4 times and it never activated once. If it did work there would have been a cooldown timer so I would know it could only happen once per 60 seconds, and I would have lived.
Nerves of Steel gives 95% Damage Reduction, not immunity. If you get hit very hard twice in a row, you will die and might not even see the icon.
For example, if you are a very low level or have bad gear, and are being carried in a GR, this will happen all the time. If a mob casts a ground effect under you, you will instantly die because 95% damage reduction is not enough to keep you from dying.

If you’re standing in a pool of nasty stuff (arcanes !!) and can’t get out of it (the 95% damage reduction only lasts for 3 seconds)… the little life you have left after Nerves of Steel proc’ed ain’t gonna last very long.
Also note that the icon(s) disappear/reset when you die.

only until you die. If it activates and you die at the same time, or shortly thereafter, chances are you will miss the icon.
Don’t know… you aren’t providing any details. You say it doesn’t work, I showed you with a screenshot that it does work. @VitaKaninen also explained how you could instantly died if you don’t have enough toughness and/or recovery to keep you alive.
You keep saying that it doesn’t work but say very little else.
Not sure what you expect Blizzard to do. There are no game logs, so they can’t load up your game and see what happened. You aren’t saying how you died. You don’t provide any video clips that would show what happened before death and that could show whether or not Nerves of Steel actually proc’ed.
At this time… you’re the only one reporting that Nerves of Steel doesn’t work.

just experiencing a few too many deaths when I went from Torment XIV to Torment XV.
When I add a new to me passive skill, I expect to see the de-buff if it activates.

You’re claiming something doesn’t work, but other players are showing it working for them, and explaining how it works to you. This means you need to provide evidence of it actually not working and, preferably, with screenshots / video showing it not working and detailing the circumstances under which it can be replicated, so we could test it too and confirm if it also doesn’t work for us. There’s plenty of people that are willing to test something if you have a scenario under which it apparently does not work.
The How to Write a Good Bug Report at the top of the forum says…
After your Summary, include a brief description of the bug you experienced. Include as much RELEVANT information as you can. Where you in multiplayer? Could you reproduce the issue? Specifics are preferred as we need to know as much as possible about the issue.
If you have steps and can reproduce them consistently, write them out. This will help Quality Assurance get a must faster turn around time reproducing the issue and reporting it to the Diablo III developers.
Finally, it always helps to attach a link to Imgur (or another picture hosting site) that displays a screenshot of the bug. If you need to include a video, link to a video as well.
