I died and exited game to reset and in the new game my main hand was just missing. I still have shield and weapon swap, but no main hand after death.
This just happened to me, exactly the same thing. No weapon, shield and weapon swap are there

The same thing just happened to me. Did NOT drop weapon and of course checked switch as my shield is still there…
Sorry to hear that. Had a similar issue happen to me but with more items. Long story short… they are just gone… Here is what blizzard told me.
