I need some help here, this is a situation I’ve never seen before.
I’m starting Act 2 and need to kill Radament to get the Hordraic scroll and get the skill point. I’ve gotten the scroll and gave it to Cane which triggers the next quest of getting the cube, the staff the headpiece, etc.
When I killed all of Radament’s minions and opened the chest I did get the Hordraric scroll. But Radament was not there. I’ver repeated the quest at least 6 times and Radament is never there. He should be on level 3 next to the chest. The chest is always there but no Radament.
Atma never says her closing dialog either, which means I haven’t killed Radament yet. She always just says she’s having second thoughts of asking me to go down to the sewers, please be careful, etc.
I’ve gone thru every inch of all 3 levels in the Lut Gholein sewers and still can’t find him. I’ve tried looking for him over and over, not just by logging out of the game but logging out of my account, then logging back in, staring a new game…
I’ve never seen it where Radament just is not there. Thanks for any help.
Are you playing offline local single player?
It may be a bugged map, and you may just need to reroll to a different map.
If you have Nightmare or Hell unlocked
Step 1: Temporarily switch difficulties and create another game. This will reroll the maps. Leave the game.
Step 2: Switch back to your target difficulty and start another game, resume questing.
If you only have Normal difficulty
Step 1: Open the Battle.net launcher
Step 2: Select Diablo II Resurrected if it isn’t already selected.
Step 3: Click the gear to the right of the “Play” button, choose “Game settings”
Step 4: Check the box by “Additional command line arguments”
Step 5: In the field that shows below “Additional command line arguments”, enter “-resetofflinemaps” without quotes.
Step 6: Click Done.
Step 7: Start another game, go check for and if present, slaughter Radament and get that skillbook.
Additionally, if the problem is resolved and you don’t want the maps resetting each time, you’ll need to go back to Step 4 and uncheck “Additional command line arguments”.
Or you can certainly leave -resetofflinemaps enabled for a random map experience every time you play to keep the play experience fresh.
3rd option if you’re tech savvy
Browse to C:\Users\username\Saved Games\Diablo II Resurrected\
You’ll see a file listing such as:
Charname.ctl
Charname.d2s
Charname.key
Charname.ma0
Charname.map
Delete the charname.ma* files.
Deleting the character map files forces the game to reroll the map.
Do NOT delete the charname.ctl, charname.d2s or charname.key files. 
If you don’t know what this means, don’t try it. I’d hate for you to accidentally delete your character.

Ok, great. Thanks for the help, I’ll give command line thing a try. I am playing offline and am still very new with this character, I’m following a Hydra Sorc build from Wowhead.
This build looks interesting, similar to the one I use with an Amazon I also play.
