Are you playing offline local single player? It may be a bugged map, and you may just need to reroll to a different map. If you have Nightmare or Hell unlocked Step 1: Temporarily switch difficulties and create another game. This will reroll the maps. Leave the game. Step 2: Switch back to your target difficulty and start another game, resume questing.
