I did two mythic + 10s and a full heroic clear last week and received 2 veteran pieces in my dungeon slots, 2 delve pieces, and 0 raid pieces when I opened my vault this week. I did have to collect last week’s reward before I opened the new vault which is how I know the new vault shouldn’t be an old reward. I don’t think I have ever even done delves on this toon, so it absolutely doesn’t make sense. I tried opening a ticket with no luck in getting any help outside of “it will most likely be hotfixed or fixed during next week’s maintenance, don’t worry”. Given that I have seen absolutely no information about a fix for this bug and I am out 2 Mythic pieces of gear and at least 2 hours of farming for them, it is incredibly frustrating. Would love for blizzard to actually have a support system that can actually fix issues when they occur rather than “don’t worry, it will probably be fixed”.
You can have more than one weeks worth of rewards backed up in the vault. When you open it, does it say at the bottom that it’s an old rewards
The delve slot isn’t just delves, it’s for doing basically anything in the open world and is very easy to unlock slots for.
So was it an older vault still?
No I was just confused, I deleted the message because it wasn’t accurate

Thank you for the suggestions, unfortunately nothing on the bottom indicates that it is an old reward. Perhaps it was due to world quests, but I also don’t tend to do anything outside of raid heal on this toon until last week when I did mythic +. I had sent a screenshot to my guild, and they confirmed I wasn’t missing something that indicated it was old rewards. Not sure if there is any other way to confirm it might be an older reward? If I opened a vault to look at it, then didn’t choose the reward, but the week rolls over and I earned new rewards, I don’t think the interface displays the message indicating it’s an old vault. Although, it being “open” immediately may be enough of an indicator in that case. But, I don’t know if the message being absent is always indicative of its status. I’m not sure if it only shows in certain circumstances, as I haven’t tested thoroughly, and also have never been several weeks behind in claiming them.
I don’t believe they would be able to fix any individual issues with your vault, anyway, so I’d say it may be best to select an item and then see if you can re-open it again afterwards. Personally, I have had some odd behavior with timewalking and delve vaults, but have not had the raid or mythic plus rewards be inaccurate.
Also, you can get rewards in the lower slots by doing open world events like the Theater Troupe, and dungeon slots for doing timewalking runs.
