I entered a 1/3 lockout from someone else to clear Azjol Nerub for the Daily Heroic quest that gives Emblem of Conquest and we changed it to Titan Rune (H+) difficulty. The popup said 1/3 upon entering so I accepted it. Then when we went further along, all 3 bosses were missing. Just not there at all.
As you can see bottom right, my lockout says 1/3 and everyone in my party just lost out on 5 Emblems of Conquest (+3 from the Satchel).
