At least the Dalaran sewers are used for under-handed activities, like secret death Arenas, magical experiments, and other such. They don’t actually live there.
Why the Forsaken chose to live in Lordaeron’s sewers when they have the entirety of the Kingdom to expand into I’ll never know.
You notice the living humans don’t have any activity within the sewers of Stormwind?

I think there’s at least 2 outhouses in game that your can actually go into. I know there’s an achievement for going into one of them
You know all those structures in cities that have doors that don’t open? They are in there. The real question is what about all the houses you can go into that just have a dining room and a bedroom, with no other doors. Where are they doing their business? My opinion, is that they just run to a nearby tree or grassy area.
