I think if a guard aggros, and hits, you you’re flagged. I was running through Duskwood, as a troll and one of the low level Alliance mobs that roam the path attacked me, setting me to pvp on.
If you stay on the left side facing the elevators, you will not get flagged going into Thousand Needles as an alliance, just stay away from the guards on the right.

Main place with unavoidable guard aggro is running through Dun Algaz to Loch Modan en route to Badlands, but I don’t think they flag PvP if you don’t damage or fight back.
I think the last part of message for the wildhammer in Hinterlands flags you.
Any NPC in the other faction’s Capitals will (and should) mark you for PvP.
They should not, but more often than not they do. It is a bug dating back to when the HC servers were first made. It has never been corrected. Blizz promised:
… attacking an enemy faction NPC will still flag you even if you haven’t typed /pvp, so steer clear of settlements of the opposing faction!

Various quests and objectives that cause players to become PvP flagged, such as the quest, “The Attack” and, “The Missing Diplomat,” no longer cause players to become flagged for PvP.
These quests were never strictly “meant” to flag you for PvP, but this was a result of how the creatures were set up at the time and was likely the result of an early technical limitation. We no longer have this limitation, and to prevent unintended consequences or “gotchas” after accepting an innocuous-looking quest, we’ve decided to no longer cause these quests to flag you for PvP combat.
Attacking, not being attacked by. But there’s several Guards flagging you just by hitting you. More than in Era, actually.
I only met the Southshore ones as a Hordie, but as an Ally I have met many Horde NPC’s flaggign you, and many of these I cheched in Era, they did not flag me.
Did you forget the Tarren Mill ones for Ally? Or am I wrong there.
