Astral Cloud Serpent. Got it on my 2nd or 3rd run through Mogu’shan, I wasn’t even farming it, I wanted a mog piece from that place.
Edit: I just recalled another that may be even rarer, the fox mount from Suramar. I got it on my first try, I actually thought it was a reward as part of the Suramar questline until a guildmate told me it was very rare to get that quest and even rarer to get the mount.
I’m not sure if it’s the rarest, but I did get Invincible to drop. It was my 13th run of the day - I had been bored and was like “I’m going to kill Arthas with all my characters”. On my final character, the mount dropped.
Honestly, the only other thing that comes to mind is Baron Rivendare’s Deathcharger, which I ended up just letting my then-girlfriend-now-ex have.
In hindsight, I wish I had ninja’d it.

I’ve got the Arcanite Ripper, which wasnt super rare or hard to get, but it only dropped for like two weeks 12 years ago. I think you can still get it on the BMAH, maybe?

Either my Sea Turtle mount (from fishing), or my Flag of Ownership toy (from the CCG). Probably those are the ones I have with highest rarity.

Shadow Priest artifact challenge appearance. Towards the end of Legion SPriests were a dying breed, so it’s pretty uncommon to see people with their artifact challenge appearance. It was once of the harder kite challenges too since Shadow only had their silence and had to use cd’s to survive the big bursts of damage.

I believe the rarest I have gotten was the archaeology fossil raptor mount .
The most expensive drop would be the trinket from WoD during launch, sold it for 150k Gold back then

serious answer is rivendares death charger, i went through that dungeon about 200 times and once i got it i was done mount farming
not just with the horse, but every mount lol
Isn’t exactly a singular rare item. But I got invincible and ashes on the same day.
I don’t think I ever have gotten anything rare lol my luck sucks rarest I guess is the brew fest mounts
The Time Lost Proto Drake.
It spawned right behind an afk Allied Night Elf hunter. Right place, right time. As I killed it and learned it, said hunter landed next to me and proceeded to /rude to me over and over again.

I’d have to say the Heavenly Onyx Cloud Serpent from the Sha of Anger. I usually never get mounts from world bosses and events, so that was a massive shock. It’s also the one I really wanted.

I was grinding out the Scarab Lord on my DK on a server that had not opened the gate yet… hen they opened all the gates so I almost got it.
I once got a piece of gear that had a type on the description… I guess that can be considered rare.

I actually have the version that says “(old2)”. It was a horde quest item but was fixed in Vanilla with an updated version that couldn’t drop for Alliance.
I don’t know why I kept it originally, but now I refuse to delete it.
Not sure exactly how rare they are, but the most rare items I have, are the Competitors Tabard, Spirit of Competition pet, Assassins Blade, and 2 Haunted Mementos.
Vial of the Sands recipe. Farmed it for a year and a half.
