You apply lots of Festering wounds to your enemies and then pop them.
What you use as the applier varies, but at the basic level it’s Festering Strike. Your spender is then Scourge Strike (with cleave by Death and Decay) or Death Coil if you get a Sudden Doom proc.

That aside just press all your CDS (you have many) which stack up every 45 seconds (Apocalypse then Raise Abomination then Dark Transformation then Unholy Assault - guide says to SSx3 to pop some wounds before using UH) .
