I still remember Sernitity Now’s infamous raid on the funeral session. Though its memorable for all the wrong reasons.
The Corrupted Blood Incident™ definitely takes first place. This is an often-repeated point, but it has been analyzed by disease specialists as a model for how epidemics can start and get out of hand quickly.
Second runner-up for me has to be the Kazzak reck-bomb that got Reckoning nerfed almost instantly.

Alvraen probably named the biggest, which was of course the Blood Plague.
Wall jumping, in my opinion, was the longest lasting bug which had the biggest impact on WoW players overall. Unfortunately they said it will not be in Classic.

That raid might qualify. Though to me, it was the epitome of the freedom to go your own way, independent of other peoples’ social norms, that world PVP was and is all about.
It would be a bigger controversy today for sure. People would demand that Blizzard ban accounts and such like they did when it happened, but it would be bigger in fact I bet it would be national news and people would call gamers heartless.
To be fair, waiting for the questions to be answered wouldn’t have made any difference in that case.

The unintended side effects of AQ opening (One of the quest steps was bugged, so the first guild to do that step got like a month long head start on the mallet, and the massive lag and constant crashing that turned the gate opening event into a massive cluster lovemaking.) When the AQ event finally comes around on Classic, I hope they limit log ins to characters 30+ or something. My server was like the third to finish the war effort, and we got such a massive number of tourists that the post gate event never really got to happen, the server kept crashing. WoW might not have peak numbers anymore, but the population is probably still higher than in vanilla. And unlike in vanilla, the other servers can’t just wait for their events.
Nobody’s gonna mention hunter pets being summoned in the auction house with the Baron Geddon bomb on them? It became an almost-regular (once-ish per hour) occurrence on my server for a month or two.
Reckoning Paladins one shotting world raid bosses was a good one. They fixed that REAL fast.
Oh, one other I thought of was when Maraudon released, they had to hotfix Mind Soothe on priests because they could run groups all the way to the bosses with minimal pulls, trivializing the entire dungeon and turning the place into a loot piñata.
