While completing WQ in Zandalar, my husband and I turned off War mode and PVP just to be able to complete them without dying every 5 minutes. But for some reason it keeps enabling it randomly.
I have both options turned off and have not returned to a Capital city to re-enable it either. But while just going from one WQ to another it randomly kicks on and I don’t realize it until we are getting attacked by Alliance. Not sure why this is happening. Logging out and back in is not helping either.
You’re getting aggro from flagged guards somewhere. Flagged guards will flag you too regardless of your WM status. And like always you’ll have to wait five minutes for the flag to drop on its own.
But Warmode off is supposed to prevent PVP, yet having player flagged as PVP for getting too close to certain npc almost seems like a bug that hostile players could exploit.
Since NPC all look the same to me, I can’t tell which one will leave me alone and which one will chase me and cause me to be flagged for PVP.

Warmode isn’t solely a PvP/PvE determiner. It’s simply mirror-copies of the game where all those who have Warmode enabled are placed together (and where there are bonuses for participating), and where those who do not are placed together. It, however, doesn’t prevent PvP in non-Warmode. Players can still be flagged for PvP with Warmode off, either by manually flagging themselves or aggro’ing a PvP NPC.
It’s necessary because if there was no way to flag for PvP, players could come and go freely in enemy cities and towns. For example, your Nelf shouldn’t be able to land in the middle of Orgrimmar without consequence.
Aside from the faction hubs, there are several places out in the world that are faction controlled. Examples would be the many faction specific flight master locations which are small towns/outposts. Moving to close to those will aggro the guards, thus flagging for PvP. For distinguishing the PvP NPCs, the mouse-over tooltip will have “PvP” included in it.

That makes a little sense. However, my husband clicked on the pet battle WQ guy and a random guard from a far away bridge ran over and started attacking him knocking him out of the battle. So I tried to help him and when I targeted the guard it said he was PvP and wouldn’t let me engage since I didn’t have it enabled. So my hubby died since he was caught off guard and in a pet battle. Then I clicked on the same NPC to start my battle and the same damn guard came after me. WTF? I couldn’t attack him before then he just flags me??
Then about 10 minutes later we were fighting a WQ elite and I couldnt’ target anyone. Once he was dead we my husband started getting attacked by an Alliance mage. But at least this time I was able to engage and assist. Just seemed odd since we were the only 3 there with no other NPCs. 
Not looking to necro, but I’ve been having something like this all day.
By myself in the valley by the Temple of Akunda…literally no other players, no guards to turn hostile.
Warmode enabled itself and I was flagged for pvp.
Then, pvp disabled and warmode turned off…at the Terrace of the Devoted. Just gliding down some stairs and minding my own business. No players I can see…and obviously no alliance/horde aligned guards.
Flagged for pvp, warmode enabled and one-shotted.(edit: not entirely sure what killed me either)
Happened in suramar earlier as well, though there was a hostile alliance player. To be fair though, they were literally sitting there not doing anything, just randomly flagged.
